Chronic Condition Information Network (CCIN) of Vermont and New Hampshire, is an independent, non profit organization, whose purpose is to assist those who live or care for those with chronic conditions so that they are able to obtain accurate and relevant information in a timely manner.

Started in 1996 as the HIVNet (Health Information Vermont/New Hampshire Network), CCIN, as it became in 2002, offers the following services:

  • Website: A free, electronic network of information and resources for those affected by chronic/life threatening conditions in New Hampshire and Vermont.

  • Pamphlets and Informational Material: CCIN continues to develop a wide array of materials that help link people to resources in the NH/VT region, including "How To Get What You Need: A Guide for Persons Living with Chronic/Life Threatening Conditions." All booklets, pamphlets and brochures are available to be downloaded for free at the Publications Section of our website.

  • News Digests: CCIN has four digests available to health care providers, caregivers, and/or people interested in having in-depth, updated scientific information, made available via e-mail.

    • SHH (STDs, HIV and Hepatitis): Posted bi-weekly on Monday, this contains the latest research on these conditions, as well as a calendar of regional events pertaining to these conditions. To subscribe, write "Subscribe SHH" in the subject heading and e-mail margoc@ludl.tds.net

    • CDIS (Chronic Disease Information Services) Digest: Posted on the 15th of each month, this digest contains the latest research on asthma and diabetes, as well as regional and on-line continuing education opportunities. This is a service of the NH Area Health Education Center. To subscribe write "Subscribe CDIS" in the subject heading and e-mail margoc@ludl.tds.net

    • Prevention Digest: Posted bi weekly on Friday, this digest is for providers who work in the field of HIV/AIDS prevention. Subscribers must be able to document that they are providers. For more information on this digest contact margoc@ludl.tds.net

    • VT M4M Digest: Posted on the 15th of each month, this health digest addresses health issues for men who have sex with men and transgender. To subscribe write "Subscribe VT M4M" in the subject heading and e-mail margoc@ludl.tds.net

    All of CCIN's publications, website and news digests are available free of charge.

  • Consulting: CCIN works with medical and social service providers in developing programs, websites, and marketing and educational materials that relate to chronic conditions.

CCIN is co-directed by Margo Caulfield and Kathy Parsonnet, both of whom have extensive experience in health information and program development. With close to 30 years' experience in health care, Ms. Caulfield's background includes strategic planning and implementation, administration, community organizing and lobbying. Ms. Parsonnet has a background in public health, education, and nursing, with over 25 years' experience in patient care, health care program development, grant-writing, and publishing.

CCIN is an autonomous, non-profit organization, and we pride ourselves in our ability to foster collaborative efforts while maintaining our independent affiliations. CCIN's Board of Directors is comprised of people who represent the populations we serve. Our activities and publications are supported through federal and state grants, Foundation and Corporate sponsors, and private donations.
