  • The Hood Center for Family Support is designed to improve the support capacities of families with members who experience disabilities and chronic health challenges; contribute to the self-determination of persons with disabilities, chronic health conditions and their families; and, establish reliable alliances between families, professionals, and communities. The Hood Center provides programs in three broad areas: Families, Youth, and Professional Collaboration. Some of programs within the Hood Center include:

    • The New Hampshire Partners in Health Project: is a comprehensive, community-based model designed to address the needs of children with chronic health conditions and their families. PIH works with both individuals and agencies that influence the family's social, educational and medical surroundings.

    • The Rural Medical Home Improvement: Rural Medical Home works with four practices located in Vermont and New Hampshire to improve their collaboration with families and to improve the care and service coordination that they need.

    • STAR (Steps Toward Adult Responsibility) is designed to help adolescents with chronic conditions develop healthy adjustment and coping skills. STAR provides both formal and informal ongoing support of adolescents including, initial consultation and assessment, individual and family sessions, and referral to community providers and resources when necessary. School consultation to support teens as they adjust to and manage the impact of their chronic illness is also available.

  • The American Diabetes Association

  • The ADA Clinical Practice Recommendations 2000

  • CHID bibliographic database is produced by health-related agencies of the Federal Government. This database provides titles, abstracts, and availability information for health information and health education resources. CHID lists a wealth of health promotion and education materials and program descriptions that are not indexed elsewhere.

  • Clinical Guidelines on the Identification, Evaluation, and Treatment of Overweight and Obesity in Adults The National Heart, Lung, and Blood Institute, in cooperation with the National Institute of Diabetes and Digestive and Kidney Diseases, released the first Federal guidelines on the identification, evaluation, and treatment of overweight and obesity

  • Clinical Trial Information The U.S. National Institutes of Health, through its National Library of Medicine, has developed ClinicalTrials.gov to provide patients, family members and members of the public current information about clinical research studies.

  • Directory of Diabetes Organizations If you want to do your own exhaustive search on Diabetes, this directory of the National Institute of Health's National Institute of Diabetes & Digestive & Kidney Diseases (NIDDK) is quite comprehensive, and lists Government agencies, voluntary associations, and private organizations that provide diabetes information and resources. Some of these diabetes organizations offer educational materials and support to people with diabetes and the general public, while others serve primarily health care professionals.

  • The Joslin Diabetes Center provides current diabetes news alerts on how to interpret information seen in the news media, along with Joslin announcements.

  • Medscape Diabetes and Endocrinology

  • The National Diabetes Education Program is a joint initiative of the National Institutes of Health (NIH) and Centers for Disease Control and Prevention (CDC). Two new NDEP Web sites are coordinated, complementary, and linked; one is managed by NIH and the other by CDC

    This program offers an on-line guide for patients, titled, "Information on Diabetes Control" as well as a listing of free publications, many of which are available in Spanish. Printed materials on how to control diabetes can be ordered online, or by calling the National Diabetes Education Program automated phone line at 1-800-438-5383.

  • National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) of the National Institutes of Health offers resources for both the provider and for the patient.

  • Pubmed is the National Library of Medicine's search service that provides access to over 11 million citations in MEDLINE, PreMEDLINE, and other related databases, with links to participating online journals.
